The Intrapartum RIsk aSsessment tool for small-for-gestational age babies is a mobile application aimed at obstetricians dealing with deliveries of high-risk fetuses. The IRIS tool has three calculators which are built for estimating the risk of operative delivery due to intrapartum fetal distress, estimating the risk of prolonged neonatal unit admission and calculating cerebroplacental ratio MoMs. The coefficients used by the risk calculators are derived from a population of suspected small for gestational age fetuses who delivered in St. George’s University Hospital in London.  Associated literature can be found in the references section. Understanding the limitations of such studies and the information they may provide is essential before starting to use the IRIS tool. Although the term “prediction model” is commonly used for such studies, it is sort of a misnomer. Rather than actually predicting an outcome which implies this calculator is optimized for your clinical setting, the “predictions” of IRIS tool are optimized for the analyzed cohort (St. George’s University of London). It is important to note that your local practices may not be accounted for in the calculations of IRIS tool. IRIS TOOL IS EASY TO USE

Six steps

IRIS tool is extremely easy to use. After selecting the risk calculator at the calculator section in main menu, we only need to enter 6 basic information about the pregnancy we are interested in. 

1. Did the parturient deliver a baby before? If so, please click yes.

2. Is the pregnancy over 39 weeks' at the time of delivery? If so, please click yes. 

3. Enter cerebroplacental ratio MoM. A calculator is provided and the button can be found at the bottom of the screen.

4. Is this an induced labor? This indicates the labor was started via induction agents prior to onset of natural contractions. If so, please click yes.

5. Is this an augmented labor? This indicates during the progression of labor, oxytoxin was started as an adjunct treatment. If so, please click yes.

6. The woman received epidural analgesia. If so, please click yes.

Then we can use the calculate button to see the individual risk estimate for this pregnancy. 

The reset button should be used each time before calculating a new pregnancy.
